Starting with the class of 2027, students are required to obtain 75 documented community service hours to graduate.

Use the form below to document any earned hours. A supervisor signature, student signature and parent signature are required. Physical copies are available in the front office and counselor's office.

Once signatures have been obtained, a photo of the form can be submitted below. Students may also turn in the completed form to the front office or counselor's office.
